6/2/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ja’Carei upon arrival to the home setting. Upon arrival to the home setting, Ja’Carei appeared to be in a positive mood evident by his facial expressions such as a smile on the face while running up to QMHP-C and providing her with his day. He verbalized to QMHP-C he did not get in trouble. QMHP-C praised him for a good job well done and encouraging him to keep up the good work. 2. Behavior: Ja’Carei was cooperative throughout the session and reported to QMHP-C that he really does listen to his grandmother all the time but he really don’t listen to his mother. 1.Intervention: QMHP-C began the session allowing Ja’Carei discuss any behaviors that were presented since the last session that QMHP-C needed to be aware of. QMHP-C explained to Ja’Carei that he must listen and follow the rule of an authority figure and not give a response if not asked a question. QMHP-C stated that is disrespectful to laugh and ignore adults. 2. Intervention: QMHP-C stated inquired if Ja’Carei had been disrespectful towards his mother (reported by mother to determine if Ja’Carei would display lying maladaptive behaviors). QMHP-C prompted Ja’Carei cease and pay attention. QMHP-C engaged Ja’Carei in a therapeutic activity. QMHP-C explained to Ja’Carei the purpose of the activity in an effort to improve his behaviors. 1.Response: Ja’Carei appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. During the session, Ja’Carei displayed fidgety behaviors laying on the floor, not listening to QMHP-C, and in a very hyperactive mood. When prompted, Ja’Carei had the ability to calm down. 2.Response: Ja’Carei stated that he does not intentionally disrespect his mother he just does not like some of the things that she says to him as well. During the therapeutic activity Ja’Carei needed several redirections from QMHP-C; however, after being redirected from QMHP-C he became more focused.
